Community Support Services


Community Support Services provide an array of person-centered support services delivered by community-based, mobile Community Support Specialists. CSS are directed toward adults, children, adolescents, and families and vary with respect to hours, type and intensity of services depending on the changing needs of each individual.


Community Support Services for adults provide assistance and support to the individual in the following activities:


~ Identification of strengths

~ Individual therapeutic interventions

~ Direct interventions to prevent crisis

~ Development of Individual Service Plan

~ Assisting individuals in determining needed services

~ Providing and coordinating Psycho-education to individuals and care givers

~ Monitoring and evaluating effectiveness of interventions to reduce symptoms


Community Support Services for children work both with the child or youth and the family in providing the following services:


~ Identifying the child or youth's needs and strengths

~ Facilitating CSS Activity Plan developed with child/youth and family and caregivers

~ Assisting the family in accessing needed services identified in the Activity Plan

~ Monitoring and evaluating the effectiveness of the interventions toward goals

~ Educating and training of family/others who play a role addressing the child's needs


Housing Support Services




Reaching out for help can be difficult and the process can be confusing.  The goal of Warren Yazoo Behavioral Health is to develop effective outreach strategies include identification of individuals in need, screening, development of rapport, offering support while assisting with immediate and basic needs and, most importantly, referral to and follow-up of those appropriate resources with respect to homelessness issues especially homeless individuals with mental illness, substance abuse, and co-occurring disorders. 

The PATH program services are provided to adults (age 18 and above) who reside in Warren County. Individuals who have a serious mental illness or a  co-occurring disorder, and are literally homeless, and/or at imminent risk of becoming homeless. Warren Yazoo Behavioral Health
 provides PATH as a coordinated and comprehensive service through its Community Support Services Program in addition to other mainstream services (including outpatient mental health therapy, nursing, medical, alcohol/drug, crisis management, employment, day treatment, and psychosocial rehabilitation services).  

Once an individual is identified the community support specialist coordinates priority services and assistance in accessing mainstream services as well as services to include crisis management, assistance in obtaining income support, supportive services, referrals for primary health services, job training, employment, and education services.  Priority is given to maximize provision of services to assist the homeless individual as soon as possible. The individuals will also be referred to the Mississippi Housing for Recovery staff member who is a Certified Peer Support Specialist for help with accessing available benefits. 


The Warren Yazoo Behavioral Health Mississippi Housing for Recovery (MH4R) program addresses housing and support service needs of persons who experience homelessness or chronic homelessness who have substance use disorders, serious mental illness, or co-occurring substance use and mental health disorders

Outreach and referrals are methods used to contact with individuals who are experiencing homelessness. The Peer Specialist offers assistance and informally assesses eligibility for MH4R services.  The Peer Specialist offers support while assisting with immediate and emergency needs including a hot meal, clothing, and hygiene kits; transportation; rental and utility deposits; and basic furnishings. The individual is assessed and referred to appropriate staff for further screening and assessment of needed services and resources needed to establish a plan for permanent supportive housing arrangements. WYBH staff and the Peer Specialist provide or ensure provision of an array of supportive services needed for an individual to live successfully in the community. The Peer Specialists and Community Support Specialists will help eligible participants to apply for mainstream benefits that include Medicaid, SSI, SSDI, TANF, SNAP, and veterans’ benefits. 

 

Warren Yazoo Behavioral Health provides support services including community support services in the home, referrals to social and primary healthcare services, crisis response services, referral to substance abuse residential at our Chemical Dependency Center and outpatient substance abuse and behavioral health services at our Outpatient Clinic.
