Community Support Services provide an array of person-centered support services delivered by community-based, mobile Community Support Specialists. CSS are directed toward adults, children, adolescents, and families and vary with respect to hours, type and intensity of services depending on the changing needs of each individual.
Community Support Services for adults provide assistance and support to the individual in the following activities:
~ Identification of strengths
~ Individual therapeutic interventions
~ Direct interventions to prevent crisis
~ Development of Individual Service Plan
~ Assisting individuals in determining needed services
~ Providing and coordinating Psycho-education to individuals and care givers
~ Monitoring and evaluating effectiveness of interventions to reduce symptoms
Community Support Services for children work both with the child or youth and the family in providing the following services:
~ Identifying the child or youth's needs and strengths
~ Facilitating CSS Activity Plan developed with child/youth and family and caregivers
~ Assisting the family in accessing needed services identified in the Activity Plan
~ Monitoring and evaluating the effectiveness of the interventions toward goals
~ Educating and training of family/others who play a role addressing the child's needs
